Who do we assist?

We work with adults and older adults who may be experiencing:

  • Neurological conditions or physical disability

  • Reduced mobility or physical changes

  • Age-related changes affecting independence

  • Difficulties managing daily tasks safely at home or in the community

Services are available to NDIS participants, Aged Care clients with self funded Home Care Packages and privately funded clients.

We are a mobile OT that services Perth’s lower South Metro region eg: City of Rockingham, Kwinana, Cockburn, City of Mandurah and surrounds.

Assessments

Assessments help us understand how you manage everyday tasks and identify supports that may enhance your safety, comfort and independence.

We provide:

  • Functional Capacity Assessments (FCA)

  • Comprehensive home and environmental assessments

  • Reports to support NDIS funding and service planning

  • Reviews to monitor changes in needs over time

All assessments are completed in your own environment to ensure recommendations are realistic and relevant.

Home Modifications
